Current medications
A list (or photo) of medications, supplements and preventives, with doses.
Diet details
What brand and how much they eat. Snap a photo of the bag.
A fresh stool sample
Especially for puppies, kittens and new adoptions. A small sample within 12 hours is perfect.
Leash or carrier
Dogs on a leash, cats in a carrier. A favorite blanket helps nervous travelers.

Jot down anything you've noticed. No question is too small.
Low-stress visits
Calmer pets, better medicine.

Leave the carrier out at home for a few days so it becomes a familiar hangout, not a warning sign.
